HOMELESS PREVENTION

If you live in suburban Cook County and are at risk of becoming homeless, call the Prevention Call Center at 877-4-COOK-15. If you live in Chicago, call the city help line at 311.

TRANSITIONAL HOUSING

If you are homeless and have consistent, verifiable income, and your last permanent address is in near-west suburban Cook County, call Leticia at West Suburban PADS (708-338-1724 or 1-888-338-1744, ext. 227) for a pre-screening application. We can help homeless individuals and families transition to a stable housing situation through subsidized rent, one-one-one case management, job and financial literacy counseling and wrap-around supportive services.

SUPPORTIVE SERVICES

If you are homeless in near-west suburban Cook County, our Support Center offers basic self-care services such as showers, clothing, storage lockers, access to computers, voice mail and mail services. We also provide access to health and case management services, and can help you obtain state I.D. cards and birth certificates, apply for entitlement benefits, complete job applications, and find affordable housing.

The Support Center Hours are: Monday, Tuesday, Thursday: 8 a.m. - 3 p.m.; Wednesday: 8 a.m. - 1 p.m.; Friday 8 a.m. - 6 p.m.
